The mission of New Mexico Quality Case Management, Inc. is to provide quality home and community based case management and community supports coordination services to individuals with developmental disabilities. New Mexico Quality Case Management is committed to promoting full community inclusion and participation; providing an environment which promotes informed choices; encouraging opportunities for independence; promoting the highest level of functioning desirable/achievable; and enhancing the overall quality of life for each of the individuals we serve.

What is Case Management?

Being Your Advocate

Your case manager is your first point of contact when you are allocated to the waiver and is your advocate, coordinator, resource, and support throughout your time on the waiver. Case Managers assist you with developing your Individual Service Plan, complete and submit your annual budget, assist with your annual eligibility, coordinate with your team, visit you in person every month, assist you to run your meetings, link you with resources and supports in your community, work with your team and state entities to ensure you have all the services and supports you need, promote independence while helping you work towards you goals and dreams, and most importantly, stands by your side as your advocate and support to pursue everything you find meaningful and important.

What are our qualifications?

Expert, Accredited Service

Our case managers have many years of experience in the fields of education, family studies, social work, and especially, the DD Waiver. Each case manager holds a degree along with having several years of individual experience in the field. Our agency is CARF accredited and has over twenty years of experience in the field that has included numerous audits and surveys from the New Mexico Department of Health. The case managers at the agency participate in ongoing training and continuing education throughout every year, and the team is constantly evolving with the times and needs of the clients we serve.

We currently serve the Metro and Northeast Regions of New Mexico. This includes the following counties: Bernalillo, Sandoval, Valencia, Torrance, Rio Arriba, Taos, Colfax, Union, Los Alamos, Santa Fe, Mora, Harding, and San Miguel.

Kristin Martin

President/Managing Director/Case Manager/Community Supports Coordinator

Hello! My name is Kristin Martin. I was born in Albuquerque, but I grew up in small town Northern New Mexico. After graduating from Escalante High School, I attended the University of New Mexico where I earned a dual Bachelor's Degree in Special and Elementary Education and a Master's Degree in Special Education. Throughout high school and college, I spent every school year as a special education classroom volunteer. While earning my degrees, I worked as an office facilitator then as a Graduate Assistant in the College of Education Special Education Department focusing on research, laws, and rights with an emphasis in severe disabilities. After completing my degrees, I worked as a service coordinator at a day program specializing in care for medically fragile clients for 4 years.
I started my career as a case manager with New Mexico Quality in 2010, and in 2016 when the original owner/director retired I took over as the new owner/director. I have a passion for advocating for and empowering our clients, and it was very important to me that when I took on the role of director, I kept a caseload and continued to work in the field with the clients. I truly enjoy watching our clients grow and excel while exploring their passions and finding their independent voice in life. It is so rewarding to be a small part of the team that promotes that evolution within those experiences.
During my down time, I enjoy spending time with my husband exploring the world. My hobbies and interests include comic books (and everything that goes with them), video games, spending time in nature (especially back home at El Vado lake), arts and crafts, gardening, books, and movies and live theatre.

Issaiah Curley

Case Manager

Hello, my name is Issaiah Curley. I was born and raised in Albuquerque, where I attended Manzano High School. After high school I choose to dive into the DD waiver and have not left. I quickly realized that this was my calling and after almost nine years I have held several positions ranging from Day Program Lead, Residential Supervisor, Job Coach, Community Support Specialist, and Guardianship Coordinator. I also work part time with New Mexico Black Leadership Council as the Special Projects Coordinator, advocating, supporting, and aiding the most marginalized populations in our city’s most affected areas. I believe that every person should have a voice and the individuals who don’t are the ones who are the most at risk; I want to be their voice. I recently went back to school last year and got my Associates in Psychology at CNM and am currently working on a bachelor’s in psychology and minor Counseling which I hope to complete by next year. After, I hope to attend Howard University to earn my Masters In Social Work.
In my spare I enjoy playing chess, reading, hanging with dogs Kalypso and Cyprus, taking care of my plant atrium, watching movies, and volunteering with the unhoused population. I also coach a Special Olympic team which I am extremely proud of. We have competed in several state competitions and won gold in Volleyball, Track and Field, Bowling, Bocce, and Softball.

Angie Treadwell

Case Manager

My name is Angie Treadwell. I was born in St. Louis, MO and grew up in a small town in the White Mountains of AZ and spent my summer's back in MO. I moved to Albuquerque in the 1990's and now consider it my home. I have worked with individuals with I/DD for over 20 years. I started in the field doing direct care at a group home while I worked my way through college. I worked in the same home with the same women for five years and I truly loved them and the job. In 2001, I earned a Bachelor of Science from UNM in Family Studies with a minor in Psychology. In August 2001, I got my first job as a case manager and have been with NMQCM since October 2005. 
The wonderful thing about Case Management is seeing how these services can change individuals lives. The families/individuals I have the honor of serving become like family to me. 
In my free time I enjoy spending time with my family, cooking, gardening, camping, canoeing, hiking, and traveling.

Rebekah Anderson

Case Manager/Community Supports Coordinator

Hello! My name is Rebekah Anderson. I was born and raised in Albuquerque New Mexico. Once I graduated from a small charter school, I began attending the University of New Mexico where I earned a Bachelor’s in Communication.
While attending UNM, I took several ASL classes where I learned about Deaf culture and American Sign Language.  
During my last year at UNM I began working as a home staff for individuals with I/DD. I also started working extra hours at a day program. Shortly after I graduated, I worked as a Service Coordinator at a day program for 2 years. I then started working as a Case Manager for New Mexico Quality in 2019.
During my down time I enjoy spending time with my husband and our friends. I enjoy playing video games and all kinds of arts and crafts. If it involves creativity, I enjoy it!

Nicole Miller

Case Manager/Community Supports Coordinator

My name is Nicole Miller. I was born in Santa Fe, NM and grew up in a small town in northern New Mexico. I completed my dual Bachelor’s degree in Psychology and Criminology from the University of New Mexico in 2006. While completing my degree I worked as both direct care staff and as a residential program manager for individuals with intellectual disabilities in Santa Fe and Albuquerque. After completing my degree, I began my career as a Case Manager and have loved empowering and advocating for individuals for the last 14 years. It’s amazing to see clients flourish given the right supports! I also have experience in guardianship, guardianship proceedings, and elderly care. I am bilingual and am fluent in both English and Spanish. I began with New Mexico Quality Case Management in January 2021 and look forward to serving you!  
In my spare time I enjoy spending time with my family and friends, working out, cooking, baking, houseplants, gardening and traveling.

Vanessa Montoya

Case Manager/Community Supports Coordinator

Hello, I am born and raised in Albuquerque, New Mexico. I graduated from Manzano High School, then I went to CNM and after a few years. I also started taking classes at New Mexico Highlands University to become a special education teacher. After a few years, I decided that I found the history of the disability community more interesting than teaching so, I decided to move to Las Vegas, New Mexico, switch majors, and finish my Bachelor of Arts emphasis in History in 2018. After graduating, I applied for the Master's program and received a Graduate Assistantship. As of December 2021, I will graduate with my Master's in Public Affairs with a concentration in History. Throughout my Master's program, I dedicated my professional paper to the history and outcome of the Los Lunas Hospital and Training School and the Jackson Class Lawsuit. I will also be presenting my paper at a few historical conferences with the hope of bringing awareness to the I/DD community. I paid my way through college by working as a direct care staff for over 8 years. When I'm not working, I love to ride my dirt bike, crochet, and work out at the gym.

Jeannetta Jim

Case Manager

My name is Jeannetta Jim. I was born in Gallup, NM and raised in the small border town of Sanders, AZ. I graduated with honors from Sanders Valley High School and continued my education at the UNM-Gallup campus. After spending two years at home and attending community college nearby, I continued to pursue my education in Human Services and Political Science at Fort Lewis College in Durango, CO. After spending about 3 years pursuing a degree, I moved to Albuquerque, and stumbled upon the DD Waiver. I began on this journey as a Direct Support Staff at a day program in 2015, working with medically fragile clients. There, I knew this path was meant for me. Shortly after, I became the Service Coordinator at the same day program. Since then, I have been advocating for our clients as a Service Coordinator with the DDW, until January of 2022. In 2022, I was given an amazing opportunity to be the Vocational Supervisor at La Paloma Greenhouse, where I ran day to day operations alongside the General Manager. This field has become my passion. It is truly amazing the many obstacles clients can accomplish when given the opportunity and investment, their accomplishments are my accomplishments and their journey has become my journey. Which is why I’ve continued this path and became a Case Manager. I began case management with New Mexico Quality Case Management in April of 2023.
During my spare time, I enjoy spending every second of it with my family. We enjoy going on weekend lake trips and exploring the different towns in New Mexico.

Lanny Garcia

Case Manager

My name is Lanny Garcia, and I was born in Santa Monica, California. I moved to Albuquerque at young age and call it home. I attended West Mesa High School, grew up playing sports, and have 1 1/2 years of college, in psychology. I happened upon working with individuals w/ IDD, after caring for my father for several years and have been affiliated with the DDW for over 18 years. I have worked as a House Manager, Residential and Service Coordinator Supervisor, Program Manager, and a Case Manager. I have served as a Youth Sports Coach and Life Coach. I have also Volunteered with Life Quest at YDDC and Friends Feeding Friends. I have a passion for helping others and being an advocate, especially for those at risk. I absolutely love to see an individual accomplish their dreams, achieve independence and experience a quality of life they deserve. In my free time I enjoy playing guitar, reading poetry/writing songs and anything pertaining to staying fit. It is my pleasure to serve you.
